The work presents a detailed analysis of mine accidents in underground as well as opencast manganese mines in India. The HFACS framework was adopted to perform the analysis and significant findings were obtained. Based upon the findings an FRA model was proposed to assess the risk level for a given situation and modify the same if found critical.
